Assertion failure in m68k assembler

I'm getting an assertion failure in cvt_frag_to_fill when running the
m68k/pcrel test.

md_convert_frag (headersP, sec, fragP);

assert (fragP->fr_next == NULL
|| ((offsetT) (fragP->fr_next->fr_address - fragP->fr_address)
== fragP->fr_fix));
Usually md_convert_frag will increment fragP->fr_fix if it had to extend
the frag due to relaxing. I have no idea how this can ever have worked
unless some changes have caused fragP->fr_next to become non-NULL
recently. Any clues?
