Stan,
I did the conversion manually.

My thought was:
- Word can output HTML
- HTML editors generally can't output Word
- Word has cross-indexes, TOC, etc.
- A doc in Word can be printed at one go (double sided, etc.); lots of HTML
pages
    don't allow this.

So, Word would be a good starting doc, and output HTML/PDF/etc. from it.

I'll finish the Word doc (basically, making it look pretty, so that 'code'
has a different look
than text, etc.) if that's what'll get maintained.  Otherwise, I won't
bother.
