C++ translation progress

Work is progressing nicely and is about 80% complete. The major problems 
were use of old-school C code and lack of consistency in typing, missing 
casts etc. The one show stopper so far is a wierd macro which won't compile 
because of a type conversion. Hopefully will fully compile and link as a C++ 
program in a day or two, will also check that it still links and builds as a 
C program.

Having crawled over much of the code, I think it will benefit from being 
reengineered as OO code.
