Sure - VAX (which uses VMS), various embedded OS which often use AOUT or 
PE formats.  HP machines use the SOM format.  I am sure that there are 
others as well.

Yes.  Well yes-ish.  ELF is the format of the future (IMHO) but these 
legacy environments are going to be around for a long time to come.
