Despite the recent changes, there was still quite some room for
improvement; some of the VexW=1 additions actually need to be
undone. See the individual patches.
1: XOP VPHADD* / VPHSUB* are VEX.W0
2: add more VexWIG
3: allow {store} to select alternative {,}PEXTRW encoding
4: fix various non-LIG templates
5: adjust {,E}VEX.W handling outside of 64-bit mode
6: adjust {,E}VEX.W handling for PEXTR* / PINSR*
7: correctly handle KMOVD with VEX.W set outside of 64-bit mode
8: correctly handle VMOVD with EVEX.W set outside of 64-bit mode
9: correctly handle VPBROADCASTD with EVEX.W set outside of 64-bit mode
