When the prefetch-fd cache was separated from the main one, the prefetch default parameters ended up with 0,0 values, unintentionally.  They should be somewhat non-zero, probably a function of the default values of the main fdcache.

    Added default value to fdcache_prefetch_mds and fdcache_prefetch_fds.
    Defaults to one half of corresponding fdcache's values.
