[BUG:2000-09-08] Miss-spelling in Dll/pthread.h and various patches

Hi there,

we are using the current pthreads-win32 build to port software to the
cygwin plattform and would like to see following things included
within the official release to make things more clean for the cygwin
side:

Besides that we will assume to contribute further cygwin based patches
too.

1. miss-spelling in pthread.h: struct ptw32_cleanup_t
2. #define statements to terminate in a single line without slashes
since they produce: stray '\' in program parse errors within
gcc-2.95.2
