[v3] libstdc++/37391

This converts the links in the pb_ds docs from
relative-to-source-directory includes to direct CVS web links. 

Although this worked for me locally, when I refreshed the online
version I still have issues. I'm not quite sure what is wrong, but I'm
working on it.
