forward_list - ext pointer support

I do not expect you to clean up patches, and my mistakes do not imply  
that.  Something went wrong with that last patch - some editor changes  
got lost - there were not supposed to be any >80 character lines.

In any event, I have gone through the entirety of forward_list, and  
corrected some 2 space violations and use of tab characters alongside  
the issues with the code I had changed (including the comma space  
issue).  This should now be in good shape.
