[v3] updates to abi.html

This isn't really what was requested in libstdc++/23773, which was to
document ABI-breaking changes.

However, this is an update to the current ABI docs. It includes current
versioning information for the gcc-3.4.x, gcc-4.0.x, and gcc-4.1.x
releases.
