ADL issues: deque

Hi,
simpler than I thought ;) Again, same issue which affected
__normal_iterator with the missing overload of operator- for equal
types, plus just a couple of missing casts to difference_type (usually
the implementation is very careful about that, in e.g., operator=,
_M_reserve_elements_at_*, erase(iterator, iterator), operator[], and
many other places).
Will wait until tomorrow, just in case I'm missing something, deque is
complex.
Tested x86-linux.
Paolo.
