setmp stuff

After fixing a bug in the x86 setjmp code I've shuffled around the
definitions in the <setjmp.h> header.  It should not cause any
problems anywhere.  If it does something is wrong with the setjmp
definitions for that platforms.  We simply never really tested that
code before.  I think the current form of the <setjmp.h> header is
what it should be.
