glibc 2.1+2.2: thread_dbP.h is not distributed

The tarball from glibc 2.1.3pre3 misses linuxthreads_db/thread_dbP.h.
I'm appending a patch to avoid this.

The patch should go into glibc 2.1.3 and 2.2.
