standard compliance changes

Hi,

I've checked in a whole bunch of more or less trivial changes in.
While reading the draft for the next Unix standard I found many
difference, most of which exist also relative to Unix98.  Not a nice
task, and it took me the whole evening and night.

Anyhow, we I announce this here is because I had to change and add
some types.  Especially the introduction of the blksize_t is
interesting since I might have screwed up the definition of struct
stat.  So please keep an eye on these problems.
