[MTASCsft PATCH WIP6 01/33] MT-, AS- and AC-Safety docs

I've just completed updating the patchset to the newly-proposed
keywords, which required very significant changes to their definitions;
their uses were also updated so as to use macros that carry slightly
more information than the expanded keywords alone.
I've also made a few changes to the *-Safe definitions, added a
Preliminary keyword, broke up the definitions into multiple info nodes,
and added a reference to the first of them to all safety notes in the
manual.
I'm not posting the other 32 patches, but if anyone's interested in
seeing how the whole thing ended up, the lxoliva/thread-safety-docs
branch was updated; d41a61440aa910781b6f276623a942e4cb1f032e is the
commit that contains all of the current changes.
