Cosmetic Updates to glibc Manual in HTML Form

None of the prototypes -- i.e. function prototypes, macro prototypes, 
variable protypes, or data type prototype -- are surrounded by the <code> 
and </code> tags.  This leads to some problems -- more inconveniences than 
problems -- when 1) parsing a file for XML and when 2) attempting to apply 
a Cascading Style Sheet (CSS) to the document.

The attached file is a universal context diff patch which makes these 
cosmetic updates to the HTML version that is entirely in one webpage.  It 
does not update the HTML documentation that is with one page per node.

These are HTML cosmetic changes only; they do not affect the actual 
documentation.
