[PATCH] Fix setcontext on ia64

I'd like to recall attention to a bug that our team reported and
patched at the beginning of this month.  In particular:

David Mosberger confirmed that the fixes are fine and should be
applied:

I bring this up because it's been almost a month and we haven't seen
any movement on this, when it seems cut-and-dry to us.  Is there
anything else we need to do to get these fixes in?
