threads and the new NPTL thread library

Not really...  I've been poking at the necessary kernel support for
some of the features and couldn't decide what to do.

For instance, trace what happens via ptrace/wait if a threaded program
exec's.  Threads can just disappear unexpectedly instead of exiting
cleanly.
