[PATCH v3 00/33] Rewrite the DWARF "partial" reader

Here is v3 of my series to rewrite the DWARF partial symbol reader.

I think this version addresses all the review comments.

This version also fixes the gdb.ada/dgopt.exp regression with fission
and fission-dwp.  Most of that fix is rolled into the new "quick"
implementation (patch #19) but some preparatory work is done in the
new patch #13.
