[PATCH 3/2] Remove dwarf2_cu::dwarf2_per_objfile

[Since I mentioned it as a possibility in the first patch, I thought I
 would do this one as well...]

This patch removes the dwarf2_per_objfile field of dwarf2_cu.  It is
redundant, since we can access dwarf2_per_objfile through
dwarf2_per_objfile, which is guaranteed to be set.
