RFA: GDB manual: consolidate reserved packet descriptions

This came out of some recent discussion on gdb@.

	* gdb.texinfo (Packets): Mention that packets beginning with
	letters are reserved once, at the top, instead of actually listing
	them all and saying "reserved".
