[patch rfc] Rationalize CALL_DUMMY_BREAKPOINT_OFFSET

Hello,

This one rationalizes CALL_DUMMY_BREAKPOINT_OFFSET and 
CALL_DUMMY_BREAKPOINT_OFFSET_P.  The former is given a default value of 
0 (removing the need for most architectures to initialize it), and the 
latter is assumed to be one (allowing it to be removed entirely).

I'll look to commit this in a day or so,
