Add AMD Flash Devices - AM29LV0xxx

Attached is a patch to add support for the following AMD flash devices:

	AM29LV081B, AM29LV017D, AM29LV033C, and AM29LV065D.

Note that a patch for the AM29LV081B support had been previously submitted,
but never commited.
