Add DHCP/BOOTP Request Tags

This patch adds the (user-defined) host name (TAG_HOST_NAME) and MAC address
client ID (TAG_DHCP_CLIENTID) to the DHCP/BOOTP request packets.  Each of
these are enabled by appropriate CDL options.
