Fix KS32C5000 Ethernet Driver MII PHY Address Support

Attached is a patch to fix the KS32C5000 ethernet driver MII PHY address
configuration option I added a few months ago.  Turns out, I forgot to
include the configuration header file in some of the PHY sources :-(
