KS32C5000 Updates

These patches were submitted before, but never applied.  Since then, several
more patches to the same files have been submitted and applied by Chris
Garry.  I'm resubmitting these patches with Chris Gary's patches merged in
(attached).

Change highlights include:
	Added configurable PHY MII address.
	Added ioctl to configure the MAC (ESA) address.
	Fixed bug that could unmask an unhandled interrupt
(CYGNUM_HAL_INTERRUPT_EXT0).
	A few other minor cosmetic tweaks.
