[ECOS] More on x86 boot loader

I've finished making changes to the boot loader so that it can load
from hard drives and hard-drive partitions.

It turned out that the reason it wouldn't boot from hard drives
straight away was a bug in the check for the end of a cylinder.  The
original check didn't honor the upper two bits as high-order track
bits.  All better.
