Log message:
	* exception.h (exception::handler_installed): Remove.
	(exception::exception): Remove old code.  Manually install SEH handler
	instead.
	(exception::~exception): For x86_64, define frame end label.
	* exceptions.cc (exception::handler_installed): Remove.
