changes for "simple" cpu sim support

Hi -

An internal simulator port uses the "simple" (i.e., non-scache) simulator.
cgen's code for this case was somewhat buggy - it generated references to
ifields via the FLD() macro that wasn't actually defined.

My changes at least define a dummy FLD() for this case.  The changes to
sim-cpu.c (-r1.5) are probably not ideal, but I didn't want to change
the control flow and validity assumptions too much.
