[PATCH 0/3] gas: #APP / #NO_APP

This is only a small initial set of changes, with the overall goal of
making -f, #NO_APP, and #APP work consistently / predictably.

1: recognize #APP at start-of-physical-line only
2: generalize / tighten #APP / #NO_APP recognition
3: maintain line numbers correctly after #APP / #NO_APP

This depends on the previously submitted "gas: correctly deal with line
comments when not preprocessing".
