[Committed][AArch64] Add support for :tlsdesc: and TLSDESC_LD_PREL19

The second of three patches adding support for tiny memory model 
TLSDESC.  This patch provides support for the :tlsdesc: modifier on the 
LDR register offset instruction along with the associated 
TLSDESC_LD_PREL19 relocation.
