[committed] Explicitly initialize all MIPS opcode fields

As promised rather too long ago, this patch explicitly initialises
each field of the mips operand tables.  It also reformats the lines
so that things line up more.  (We blew the 80-column limit years ago.)

Tested on various targets and applied.

	* Makefile.am (mips-opc.lo, micromips-opc.lo, mips16-opc.lo): Remove
	special rules.
	* Makefile.in: Regenerate.
	* mips-opc.c, micromips-opc.c, mips16-opc.c: Explicitly initialize
	all fields.  Reformat.
