[patch 4/9] Nios II gas support, V2

This is a revised version of the patch originally posted here:

In this version, I've fixed the compilation errors noted by Nick:

and made the changes to the documentation requested by Joseph:
http://sourceware.org/ml/binutils/2013-01/msg00383.html

Joseph also suggested testing on a 64-bit host, which turned up a 
hard-wired 32-bit width assumption in md_apply_fix, which I've fixed. 
Additionally I attempted to clean up the spaces/tabs issues in the new 
.c files.

Is this version OK to check in?
