include/*/*.h need GPLv3 updates

I noticed that files in include/{aout,coff,elf,nlm,opcode}/ are still 
generally under GPLv2 - or in some cases GPLv1.  These files have various 
notices identifying them as parts of Binutils, BFD, GAS, GDB, etc.; 
whichever component of the toolchain they refer to, I believe they should 
be GPLv3.

There are a few files with FSF copyright notices but no license notice, 
that should have standard GPLv3 notices added, a few that are already 
GPLv3 and so don't need updates, and at least one (coff/sym.h) with a 
non-FSF copyright notice that should not be changed.

None of these files are included in the GCC repository so there is no need 
to sync updates between repositories.
