reapply patch to fix index.html/Index.html naming conflict

It appears that this change to the Binutils manual:

* doc/binutils.texi: Update to FDL 1.2.
   (Top): consistent punctuation in menu entries.
   (Index): Rename from Binutils Index.
* doc/fdl.texi: Update from http://www.gnu.org/licenses/fdl.texi.

perhaps unintentionally reverted this previous patch to prevent a filename 
conflict on case-insensitive filesystems:

Nick, since you approved the renaming to "Binutils Index" previously, is it OK 
to reapply Carlos's patch?  It's still needed for the same reasons brought up in 
the original discussion.
