[PATCH, ARM] Use unified syntax for disassembling VFP instructions

The current code for disassembly of VFP instructions is a bit of a mess.
Some of the time the new unified syntax is used and others the old
syntax is used.

This patch fixes things to consistently use the new unified syntax, and
adjusts the testsuite for these changes.
