frv warning fix, testcase update

Alan mentioned he got a warning compiling elf32-frv.c with GCC4.
While fixing that, I noticed we had some messages with duplicate
warning:, and some error messages that had warning: printed next to
them, so I fixed it as well, and updated the testcases.  I also
noticed a testcase had been left behind in the last round of updates
(or so it seems), so I fixed that as well.  Here's the patch.
