[PATCH] clean up gas documentation

Here are some changes to fix up a number of issues in the documentation for 
GAS.  Some of these are similar to the patches I've sent for other 
documentation.  One problem unique to GAS is that the current docs are 
slanted toward COFF and a.out formats; support for the ELF format is not 
described well, especially if you try to produce an ELF-only version of the 
document.  I've fixed some of the most obvious places where this is  a 
problem, but there is a lot more that could be done.  I'm not going to do it 
now, though, since I need to work on submitting the Xtensa port.
