[PATCH] Clean up ld docs (part 1)

This patch cleans up the ld documentation in much the same way as my patches 
for binutils and bfd docs.  I'm splitting this into 2 patches so that it is 
more clear what is changed.  The second patch moves the entire WIN32 node, 
because it is currently stuck in between two Hitachi-specific nodes and the 
menus show that it should be positioned after the TI COFF node.  I'm going to 
send the second patch separately.
