[PATCH]: Document missing parts for 68HC11 assembler

Hi!

I committed this patch to document:

   - some HC11 specific options (already defined in usage part),
     (also fixed to use the same look as other ports)

   - the 68HC12 specific operands

   - a new section for %addr, %page, %hi, %lo operand modifiers

   - a new section for the assembler directives (.mode, .far., .interrupt, ...)

Verified on as.dvi, as.ps, as.info and ran ispell.
