Fix for tc-s390.[ch]

Approved and applied.

Note - I fixed up the ChangeLog entry to follow the current
conventions.

Note - I also fixed up the formatting of the tc-s390.c file to follow
the GNU coding conventions.

Note - I also added a new function: s390_force_relocation and used
this to force relocations to be generated for the VT_ relocs.  This
fixed a failure in the GAS testsuite, so that now the s390 target has
no gas testsuite failures.
