XEP does and I used it in the latest 1-up renditions of our XSLT/XPath and 
XSLFO books (the 2-up renditions are meant to be printed and not browsed 
through the embedded hyperlinks).  The 1-up free download previews 
available from the web site also include the PDF bookmarks.
