My output is still missing the second question mark.  The following works:
<xsl:template match="php-echo">
   <xsl:processing-instruction>
      echo '<xsl:value-of select="text()"/>';
   ?</xsl:processing-instruction>
</xsl:template>

But why doesn't that second question mark appear automaticly?
