Dear Jeni and others!

First: Jeni is right about the semantics, but when it comes down to
performance, this is what works the best.

	<xsl:text disable-output-escaping="yes">&lt;option</xsl:text>
		<xsl:text> value="</xsl:text>
			<xsl:value-of select="Effect"/>
		<xsl:text>"</xsl:text>
	<xsl:text disable-output-escaping="yes">&gt;</xsl:text>
		<xsl:value-of select="Display"/>
	<xsl:text disable-output-escaping="yes">&lt;/option&gt;</xsl:text>

I have done some performance test, which shows some significant improvements
when You do it like above.

Or, alternatively, an attribute-value template.  Place the 'select'
expression of the xsl:value-of that you want to be able to write in curly
braces (e.g. {Effect}) within the attribute to achieve the same effect.

<xsl:template match="Code">
    <option value="{Effect}">
        <xsl:value-of select="Display"/>
    </option>
</xsl:template>

There's also no need to use xsl:element unless you want to.  It works
perfectly well to do:

<xsl:template match="Code">
    <option>
        <xsl:attribute name="value">
             <xsl:value-of select="Effect"/>
        </xsl:attribute>
        <xsl:value-of select="Display"/>
    </option>
</xsl:template>
