True, and I therefore apologize for replying to the list, Tony, but...

...there is a simple answer, and it's not related to DocBook, so
I thought it might be better to make the answer here.

OASIS (at the time, SGML Open) "wrote the book" on the CALS Table
Model (the folks at CALS even adopted this as their official
semantics), and it's available at:

http://www.oasis-open.org/html/a502.htm

(though the conversion of this document from SGML to HTML was pretty bad, 
so the format of this document has problems, but the content should still
be decipherable).

For any further info/comments/response, please take it off this list
per Tony's request.
