I mistakes writing source of the problem.
The problem is caused from _int_malloc() function in malloc.c
(glibc 2.7 from apt-get install glibc-source in Ubuntu 8.04)
4111 *fb = victim->fd;
victim->fd must be reset like 'victim->fd = NULL' after above line.
