Then how about including the command name, or a virtual command
name like "repeat (@key{RET})"?

Hmmm. The combined index isn't really that large, less than 10
pages, and it doesn't seem too hard to find the command names
in it. I think we'd want to think about it at the 20-30 page
level, or if concept entries massively outnumber command entries.
