Just to follow our discussions on the header and footer, I do find that
the alignment directive in the header/footer blocks is being ignored
with all of the columns being left aligned in the pdfxmltex output.
Also, /every/ page is precisely the same amount too tall -- I increased
my margins so it no longer looks ugly, but we still get the following
message repeated on every page:
Overfull \vbox (5.96419pt too high) has occurred while \output is active
