I'll try the fixed width and report back -- I tried to fix the width
fo the table from 100% to 6in but that gave the same result; the columns
are compiling now (old computer).
Also, I don't know if this is related, but xsltproc gives me many of the
following errors, which seem benign but perhaps not:
titlepage.fo.xsl:4711: error: Empty namespace name for prefix ns80
