I'm pretty sure this is a PassiveTeX bug. I included it
in a list of problems I sent to Sebastian Rahtz.
Some but not all of my headers in one document have this
problem. The header/footer rules are drawn as borders on
one edge of the fo:table that is used to format the line.
The current PT seems to have some problems with table
borders, as the left border on tables is also missing.
